HEALTHY BABY CONTEST HAWAII
Thank you for your interest in competing in this year's Healthy Baby Contest! Once registered, you will receive an email with the instructions below on how to record and upload your video to us!

A. Video Submission
1. Create a creative 30 to 45 seconds video of your keiki to include yourself narrating your keiki’s name and answering “my keiki is the healthiest baby in Hawaii because....”
***Please note that any video submissions over this time limit will be deducted points for every additional 15 seconds.***
2. Please ensure that your video is in MP4 or MOV format and titled with ChildLastName_FirstName
3. Upload your video to our HBC Video & Waiver Form Upload Page.
B. Waiver Form
4. Please complete the Release & Waiver of Liability form that can be downloaded here.
5. Parent/Guardian Signature needs to be hand signed in black or blue ink or signed digitally.
6. Please retitle your form to ChildLastName_FirstNameFORM
7. Upload your waiver to our HBC Video & Waiver Form Upload Page
